Insurance at Form Recovery and Wellness
Form Recovery and Wellness accepts insurance in two ways: direct billing with some insurers, and payment at the clinic followed by a reimbursement claim with others.[1] Insurance can be used for physiotherapy, Traditional Chinese Medicine and Applied Thai Traditional Medicine treatments, subject to each patient's policy.[1][2]
Direct billing
[edit]With direct billing, the clinic settles treatment fees directly with the insurer, so covered patients do not pay the covered amount up front.[1]

The clinic states that direct billing applies to approved treatment and depends on the patient's policy and insurer approval.[4]
Cigna process
[edit]The clinic describes four steps for Cigna patients:[1]

| 1. Eligibility check | The patient visits or contacts the clinic to confirm coverage for the treatments needed, bringing the Cigna insurance card. |
| 2. Consultation and plan | A practitioner carries out a consultation and writes a treatment plan. |
| 3. Guarantee of Payment | The clinic submits a Guarantee of Payment (GOP) request to Cigna Global. |
| 4. Treatment | Once the GOP is approved, sessions start without out-of-pocket payment. |
The clinic lists physiotherapy, acupuncture, dry needling, cupping, acupressure and lymphatic drainage among the treatments for which Cigna coverage can be checked.[1]
Other insurers
[edit]Patients whose insurer is not on the direct-billing list pay at the clinic and then claim from their insurer. The clinic provides the required paperwork, including medical certificates. It states that it has helped patients claim reimbursement from insurers including AETNA, BUPA, LMG, AIA and AXA.[1]
International patients
